BEFORE WE LEAVE FOR CHICAGO, PLEASE COMPLETE A RELEASE FORM. YOU WILL NOT BE ALLOWED TO TRAVEL TO CHICAGO WITOUT COMPLETING & TURNING IN THIS FORM.

Please meet at St. James the Greater Parish (1360 Tamm Ave, 63139) at 5:30 am on Saturday morning. We will plan on being on the road by 5:45 am. You should bring breakfast, snacks, drinks, etc as we want to minimize our stops on the way up there. Our game schedule is below. We plan on leaving the pool immediately after the final game and will either eat prior to getting on the road or will stop for a quick bite.
The coach going on the trip is Charlie Cutelli (314.541.4704). He will be driving along with the Meyer family. If you need to get a hold of me please feel free to call my cell phone.
I am looking for another parent driver to drive three more athletes. If you can help out I would appreciate it.

Cost of the trip per athlete: $55.00 (Please make check payable to Charlie Cutelli). Lunch, dinner, etc. are not included in this fee.
